Risk Assessment, Behavioral Evaluation, and Training Agreement
This agreement applies to any and all dogs under the care of my organization, which My Sidekick Dog Training, LLC has or will have assessed, evaluated, or trained. Please carefully read the following before initialing or signing.
My Sidekick Dog Training, LLC cannot make any guarantee regarding the future behavior and outcome of any dog. By initialing below, I assume full responsibility for the behavior of dog(s) under the care of the organization I represent, during and after any assessment, evaluation, or training.

By initialing below, I acknowledge that the dog(s) under the care of the organization I represent may have or develop aggressive behaviors. My Sidekick Dog Training, LLC has fully explained that since the outcome of any assessment, evaluation or training program cannot be guaranteed and aggressive behavior is or may become a risk to people and/or other animals, my organization’s decision to adopt out any dog is done solely and completely at my organization’s discretion with my organization taking 100% responsibility for any damage the dog(s) might cause to anyone during or after the assessment, evaluation, or training. My Sidekick Dog Training, LLC does not promise to eliminate or curtail any dog’s aggressive behavior. I further agree to take all possible safety measures recommended, including but not limited to muzzling and leashing dog(s) when around people and/or other animals.

By initialing below, I understand that My Sidekick Dog Training, LLC offers risk assessments, evaluations and training at a discounted rate of $50/hour to rescues and shelters. This can be paid in cash (preferred), or via Venmo: @ashrose313, Cashapp: $mysidekicktraining, or Paypal: mysidekicktraining@gmail.com (select friends and family to avoid being charged the 20% fee). My Sidekick Dog Training, LLC does not offer free services unless otherwise discussed.

My Sidekick Dog Training, LLC is committed to furthering the mission of reward based training by providing free education to aspiring dog trainers. By initialing below, I understand that any sessions may have 1-2 people shadowing the trainer. They are there to learn from the trainer by quietly observing and taking notes, and will not interfere with the assessment, evaluation or training session.
The terms and conditions set forth in this agreement cannot be modified or changed in any way unless agreed to by both parties in writing. By initialing below I am indicating that I have read, fully understand and agree to all of the above contract terms.

By SIGNING MY NAME below, I as a representative of the legal owner/agent of the organization listed above, having carefully read and fully understanding this agreement, do hereby waive and release My Sidekick Dog Training, LLC from any and all liability of any nature. This includes any injury, death, sickness or damage the dogs may suffer during or after any assessment, evaluation or training session. I also agree to indemnify and hold harmless My Sidekick Dog Training, LLC from any and all claims due to damage the dog(s) may cause to any person and/or animal during or after assessment, evaluation or training.
